Measure: Percentage of patients 65 years of age and older who have ever received a pneumococcal vaccine.
Numerator: Patients who have ever received a pneumococcal vaccination
Denominator: Patients 65 years of age and older with a visit during the measurement period.

In order to qualify for this measure, the provider must have seen the patient (age 65 and older) at least one time during the reporting period and have the appropriate information documented in the chart:

There are no exclusions for this measure.

ADDITIONAL INFORMATION:

•  It is recommended to use the immunization procedure code when documenting the status of the Pneumococcal Vaccination.   It must contain the date that the vaccintion was done in the procedure order date.  It is recommend to include this in a health maintenance/preventive care procedure checklist.

•  If the vaccination was not done in your office or if the record is being transferred from paper to electronic form, use the Historical check box on the immunization procedure screen:

•   The only data used to determine the denominator is data from the ChartMaker Clinical Module. If a patient encounter was not entered into the ChartMaker Clinical Module, that encounter is not included in the denominator for the statistical calculations on the Meaningful Use Dashboard. Please add these additional patients to the denominator and recalculate the percentage for Attestation purposes
