Clinical: Chart > Export > Transition of Care All Patients

What do I need to know?

The Transition of Care All Patients export functionality has been updated to allow you to export Transition of Care (TOC) documents by Practice and for a Note Activity date range, if applicable. When selecting Chart > Export > Transition of Care All Patients, a new Transition of Care Export dialog will appear allowing you to select which Practices to export TOC documents for, as well as the ability to select a specific time frame for Note Activity.

All practices will be selected by default, and you can uncheck any practices that should not be included in the export. If there is only a single practice that the user is associated with, it will be checked, and the selection ability will be disabled. If a Note Activity date range is not selected, the entire patient population will be exported for the selected practices. If a date range is selected, then only patients for the selected practice(s) that have a note in their chart with activity within that timeframe will be exported.

How do I use it?

Upon upgrade, this functionality will be available as outlined above. Simply access the Transition of Care Export dialog, be sure that the applicable practices are checked, configure any dates for the Note Activity, if applicable, and then click the Export button.

Clinical: Reports > MIPS Dashboard

What do I need to know?

The MIPS Dashboard has been updated to default to the 2024 view when initially accessed, and prior to selecting a Configuration. Likewise, the default percentages of the MIPS total score will be displayed for 2024: Quality at 30%, Promoting Interoperability at 25%, Improvement Activities at 15%, and Cost at 30%. Do note, that once the configuration is selected for 2024 reporting period if you are exempt from the Promoting Interoperability category, the 25% will be reallocated to the Quality category; and if the Cost measures are not met, the 30% will be reallocated to the Quality category.

Also, for 2024, the dashboard has been updated to calculate the estimated MIPS total composite score, as well as Print and Generate File for Submission that includes all categories.

In addition, the title bar will display the reporting year, defaulting to 2024 if no configuration is selected. Once a configuration is selected, the reporting year will be dependent on the period configured for the selected configuration. Also, when accessing individual category dialogs (Quality Measures, Promoting Interoperability, Improvement Activities, and Cost) the reporting year will also appear in those title bars following the configuration name.

The MIPS Category Requirements dialog, accessed via the MIPS Requirements link in the MIPS Dashboard, has been updated to reflect the new category requirements for the 2024 reporting period.

In addition, the MIPS Dashboard Configuration dialog has been updated for the MIPS 2024 reporting period. The functionality and various configuration options are the same as 2023, with the exception that the performance period must be at least 180 days for 2024. If the dates configured for the Performance period is under 180 days, a warning message will appear when clicking the OK button to exit the dialog.

Clinical: Reports > MIPS Dashboard > Quality

What do I need to know?

All the Quality Measures for MIPS 2024 in the Quality Measure dialog have been updated for 2024 reporting period, and for the 2024 performance period will be 30% of the MIPS Total Score (unless PI and Cost categories are reallocated to Quality category). Likewise, all the associated benchmarks for each CQM have been updated to the 2024 version, which can be accessed by double-clicking each individual measure in the right pane.

Five CQMs have been deprecated for the Traditional 2024 MIPS reporting period: CMS 69 – Preventive Care and Screening: Body Mass Index (BMI) Screening and Follow-Up Plan, CMS 125 – Breast Cancer Screening, CMS 127 – Pneumococcal Vaccination Status for Older Adults, CMS 130 – Colorectal Cancer Screening, and CMS147 – Preventive Care and Screening: Influenza Immunization. These CQMs are now grouped under a Not Available for Traditional MIPS section on the left and can still be used for other reporting programs (i.e., CMS 69, CMD 125, and CMS 147 can be used for certain MIPS Value Pathways (MVPs) reporting, while all five of the CQMs can be used for NCQA PCMH (Patient Centered Medical Home) reporting).

Also note that, for MIPS 2024, this year (and every year) CMS has made changes to the requirements for the majority of the CQMs. Please be sure to check the CQMs you are reporting to determine if changes were made that may affect your reporting and adjust accordingly.

In addition, the QRDA III file format that is generated via the Generate File for Submission button, has been updated for the 2024 reporting period, allowing you to successfully submit files using the 2024 version of the eCQMs.
