Measure:
Percentage of patients 3-17 years of age who had an outpatient visit with a Primary Care Physician (PCP) or Obstetrician/Gynecologist (OB/GYN) and who had evidence of the following during the measurement period. Three rates are reported. - Percentage of patients with height, weight, and body mass index (BMI) percentile documentation - Percentage of patients with counseling for nutrition - Percentage of patients with counseling for physical activity
Numerator (BMI): Patients who had a height, weight and body mass index (BMI) percentile recorded during the measurement period.
Numerator (Counseling for Nutrition): Patients who had counseling for nutrition during a visit that occurs during the measurement period.
Numerator (Counseling for Physical Activity): Patients who had counseling for physical activity during a visit that occurs during the measurement period.
Denominator: Patients 3-17 years of age with at least one outpatient visit with a primary care physician (PCP) or an obstetrician/gynecologist (OB/GYN) during the measurement period
Denominator Exclusion:
Patients who have a diagnosis of pregnancy during the measurement period. Exclude patients who were in hospice care during the measurement year.

EXCLUSION DETAILS:

Exclusions include patients who have a diagnosis of Pregnancy during the reporting period. In order to meet the exclusion requirement, the appropriate information needs to be documented in the chart:

ADDITIONAL INFORMATION:

•  The patient must be at least 3 years old and younger than 17 prior to the start of the measurement period.

• Because BMI norms for youth vary with age and sex, this measure evaluates whether BMI percentile is assessed rather than an absolute BMI value.

•  The SNOMED codes required for the Counseling for Nutrition and Physical Activity numerators must be attached to a procedure code other than the office code.  If a valid CPT code exists for these purposes, it may be used to attach the SNOMED codes.  However, if there is no valid CPT code, a new In House (non-billable) code can be created for documentation purposes.

•  SNOMED codes must be attached by the user when in a note.

•  The only data used to determine the denominator is data from the ChartMaker Clinical Module. If a patient encounter was not entered into the ChartMaker Clinical Module, that encounter is not included in the denominator for the statistical calculations on the Meaningful Use Dashboard. Please add these additional patients to the denominator and recalculate the percentage for Attestation purposes.
