PatientPortal Enrollment and/or Deactivation

It is important to read the following instructions to ensure your enrollment is processed correctly. To register, click the REGISTRATION tab below; to deactivate a provider, click the DEACTIVATE tab below.

Information on PatientPortal Registration:

The ChartMaker® PatientPortal is an online service that allows a patient to keep track of their personal medical information. It will also allow practices to share and receive information easily with patients. Please complete the following form to start the process of registering your practice with the ChartMaker® PatientPortal service. There is a fee of $29/physician, $15/NP, $15/PA per month.

NOTE: This is a practice enrollment form. Each provider in your practice will be added to the PatientPortal accordingly.
