ChartMaker® Clinical provides the clinician with clinical flow sheets which can be added to or modified to assist the clinician with Decision Support and various Guideline reminders while providing an overall framework to provide a graphical representation of procedures and/or results to be tracked. Upon initial install of ChartMaker® Clinical, we provide new clients with predetermined health alerts and rules via flow sheets for immunizations, adult preventive medicine, diabetes and vital signs. These flow sheets were developed over time following rules from the CDC, www.guidelines.gov, STI medical personnel and client feedback. The clinical content and health alerts are then maintained and updated by the clinician, who relies on various sources including CDC publications and information obtained from www.guidelines.gov.
The flow sheets assist the clinician with decision support reminders. These flow sheets can be modified to remind the clinician of needed or recommended procedures through visual indicators, based on age and/or gender.
The flow sheet contains the Procedure name, the Required time interval
at which the procedure is due, the time since it has been done with a
visual indicator as to whether the procedure was ever done, whether it
is due and the date on which that procedure was completed.
When applicable, the system will automatically update information in patient
flow sheets with data contained in imported lab reports.
An example of a flow sheet is shown below:
The Procedure is listed on the left followed by the Required time interval between the tests, The time since last completed and the date on which the procedure was last completed. The yellow blocks indicate that the procedure is due. The Question Mark indicates that the procedure has never been documented as being completed.
When in another part of the patient chart, the
clinician will see a Health Alert button at the bottom of the Face Sheet
(Summary Sheet) that changes color when a procedure is due based on age
or gender. That button will turn yellow to indicate that a procedure is
due. When the procedure has been satisfied it will become no longer be
highlighted (yellow).
