NJIIS Enrollment

To enroll with NJIIS you must complete the online interface enrollment. Use the following steps to do this. If you have any issues completing the form, contact NJIIS Helpdesk at (609) 984-0224.

1. Go to https://njiis.nj.gov/njiis.
    

2. Click NJIIS Documents (on the left).
    

3. Click Online next to NJIIS Interface Enrollment Request Form.
   
   
    

4. Complete the entire form, using the table below to enter the fields listed.

5. Print the confirmation to keep a copy for your records.
   
   NOTE:  Your Enrollment Form Number will be on the confirmation, as well as emailed to you. You will receive several email confirmations back from NJIIS with the status of your submission. Keep a copy of those emails for your records as well.
   
   You will be prompted to submit a Smoke Test file. Please contact your STI representative to complete this step.
    

6. Sign up for NJIIS Immunization Training. Go to https://njiis.nj.gov/njiis/jsp/trainingschedule.jsp, s0croll to the bottom of the page and select Pre-Registration next to the NJIIS Advanced Training Webinars. Select Interface Webinar and answer two required questions pertaining to Interface when pre-registering.
   
   NOTE: You are required by NJIIS to attend training before you will receive a username and password to use the online interface. Please notify STI once you have completed training.
